Your first visit to Dermatology and Cosmetic Center of Oklahoma involves a few special steps so that we can get to know you. To understand what to expect, please read through the information and forms on this page. You'll find all the practical information you need, such as a map and directions to our office, office hours, payment policies and more. You can also save some time by completing the patient forms via our online portal in advance of your appointment. A link to the portal will be sent at the time of scheduling.

Online Patient Forms
Please visit our online portal at dermcenterok.ema.md to complete required information prior to your visit. You will be given information on how to access the portal when you schedule your appointment. By completing these forms ahead of time it will greatly expedite the check-in process.
Appointments
We ask that you arrive for your appointment 10 minutes prior to your scheduled time. This allows for adequate time for check-in. Please bring your driver's license and current insurance card to every appointment.
Late/No-Show/Cancellation
In order to provide the best care to all of our patients if you are going to be more than 5 minutes late for your appointment we ask that you please notify the office to ensure that we can still work you in. If you need to cancel your appointment we ask for a 24 hour notice. If you do not provide a 24 hour cancellation notice you will be charged a $25 fee. If you are scheduled for a surgery, cosmetic consultation or cosmetic procedure and do not provide a 24 hour cancellation notice you will be charged a $100 fee.
Financial Policy
Patients are responsible for payment of any applicable copay at the time of service. All cosmetic procedures must be paid in full at the time of service. There will be a $30 fee for returned checks.
Dermatology and Cosmetic Center of Oklahoma is contracted with many insurance companies and will file all claims on behalf of the patient. It is the patient’s responsibility to verify that we are in network with your insurance company prior to your appointment. In the event your health plan determines a service to be “not covered”, or you do not have an authorization, you may be responsible for the complete charge. Patients are encouraged to contact their insurance company for clarification of benefits prior to services being rendered.
If Dr. Doty performs any of the following: biopsy, injection, curettage (scraping) and electrodessication (burning), freezing, or excising a lesion or growth, your insurance company will consider it an office surgery or procedure. Such outpatient surgeries and procedures are often subject to separate co-pays
and/or deductibles. Please check with your insurance carrier to determine how your benefits apply.
At the time of your appointment, we ask that you place a credit card on file for any balance that may remain after insurance pays. We will notify you by a statement in the mail for any balances due. If the balance reaches 30+ days, the credit card we have on file will automatically be charged for the entire
amount.
You may receive a separate bill for laboratory or pathology services from an off-site lab. If you have questions regarding this bill, please contact them directly.
